Insurance
I am in network with Blue Cross and Blue Shield PPO/BlueChoice PPO.
I am also an out of network (OON) provider. Many insurance companies provide OON benefits that can help with costs.
Medicare clients: As an LCPC in IL, I am neither allowed to panel with Medicare, nor to provide services to clients with Medicare coverage.
I will be happy to check your benefits for you before our first appointment if you are uncertain of your coverage.
- The simplest way is to enter your information (including date of birth) and upload a photo of the front and back of your insurance card into your client portal.
- You can also text or email me the above information if you would like to know your coverage before making your first appointment.
If you have HMO insurance, I will provide you with a super-bill that you can send to your insurance company.
If you would prefer to self-pay rather than use insurance, it is always an option.
*Please note that benefits are an agreement between you and your insurance company. I cannot guarantee any insurance coverage or reimbursement. If you have eligibility concerns or questions, you should contact your insurance provider directly.
Rates
Individual Counseling
$180 – Diagnostic Interview (first session)
$150 – Remaining sessions (50 mins)If you are using your BCBS PPO insurance, I will honor the contracted rate per session while you are covered.
No Shows | Late Cancellations
No-shows and late cancellations will be charged the full session rate. Insurance does not cover fees for missed appointments.
